Who Needs Orthognathic Surgery?

Improper biting of upper and lower teeth can be corrected by Orthognathic Surgery. Jaw growth is a gradual process and in some instances, the upper and lower jaws may grow at different rates. The result can be a host of problems that can affect chewing function, speech, long-term oral health and appearance. Injury to the jaw and birth defects can also affect jaw alignment. Orthodontics alone can correct bite problems when only the teeth are involved. Orthognathic surgery may be required for the jaws when repositioning is necessary.

Wisdom teeth are the last teeth to erupt within the mouth. When they align properly and gum tissue is healthy, no treatment is needed.  Extraction of wisdom teeth is necessary when they are prevented from properly erupting within the mouth. They may grow sideways, partially emerge from the gum, and even remain trapped beneath the gum and bone. Impacted teeth can take many positions in the bone as they attempt to find a pathway that will allow them to successfully erupt.

These poorly positioned impacted teeth can cause many problems. When they are partially erupted, the opening around the teeth allows bacteria to grow and will eventually cause an infection. The result: swelling, stiffness, pain, and illness. The pressure from the erupting wisdom teeth may move other teeth and disrupt the orthodontic or natural alignment of teeth.

Removal of the offending impacted teeth usually solve these problems. Early removal is recommended to avoid such future problems and to decrease the surgical risk involved with the procedure.

TMJ (temporomandibular joint) disorders are a family of problems related to your complex jaw joint. If you have had symptoms like pain or a “clicking” sound involves TMJ problems. These problems are more easily diagnosed and treated than they were in the past. These symptoms occur when the joints of the jaw and the chewing muscles (muscles of mastication) do not work together correctly. TMJ stands for Temporomandibular Joint, which is the name for each joint (right and left) that connects your jaw to your skull. Since some types of TMJ problems can lead to more serious conditions, early detection and treatment are important.

Cleft Lip & Cleft Palate are the defects present at birth with the opening in the upper lip and roof of the mouth (palate)

CLEFT LIP :

  • a cleft lip happens if the tissue that makes up the lip does not join completely before birth. This results in an opening in the upper lip. it varies from  small slit to a  large opening that extends  into the nose. a cleft lip can be on one or both sides of the lip or in the middle of the lip, which occurs very rarely.

 

  • CLEFT PALATE:

A cleft palate happens if the tissue that makes up the roof of the mouth does not join together completely during pregnancy. 

These children will have problems with feeding ,speaking and can have ear infections. they also might have hearing problems and problems with their teeth.

 treatment varies  depending on the severity of the cleft; the child’s age and needs; and the presence of associated syndromes or other birth defects, or both.

  • timing of the lip surgery varies from first 48 hours of life to 6 months of age.
  • surgery to repair a cleft palate is recommended within the first 18 months of life or earlier if possible.
  • surgical repair can improve the look and appearance of a child’s face and might also improve breathing, hearing, and speech and language development.

Distraction osteogenesis is a biologic process of new bone formation between the surfaces of bone segments that are gradually separated by incremental traction.

  • useful and well-established technique for bone and soft tissue formation in moderate to severe bone deficiency cases, like deficient maxilla or midface in cleft palate , deficient hypoplastic mandible in  congenital craniofacial malformations such as Pierre-Robin sequence, deficient alveolar bone prior to implant placement, TMJ ankylosis.
  • It is a promising technique mandibular DO for children with micrognathia .
  • Mandibular continuity defects resulting from excision of tumors and/or aggressive developmental cysts can be treated with this.

Distraction osteogenesis includes four steps:

  1. Corticotomy or osteotomy and placement of the distraction devices
  2. Latency period of several days for callous organization
  3. Gradual distraction at a rate of 0.5–1 mm per day
  4. Retention/consolidation period of several months for callus maturation and mineralization.
